If you would like to communicate with our staff regarding the status of a child’s referral, please have the parent or caregiver sign a sign this release of information form and upload it with the referral.

The release of information form is optional. However, without parental consent, our ability to share updates about the child’s referral will be limited.

Please note: You do not need to complete this form if the child is enrolled in a Head Start or district-run preschool program, as existing agreements allow us to exchange educational information. This request applies only to children enrolled in other community preschool programs and child care settings.

If you choose not to obtain a signed release form, we ask that you inform the family that you have made a referral on their child’s behalf, so it is not a surprise when we contact them. We will only move forward with the process if the family chooses to participate.
